Child Development Documents

Use these documents to help develop your children.

These documents include:

1. Focus Week Observation (Weekly observation sheet – with example) – focus weeks observations are a detailed observation to get a review of the child’s development.  These are introduced to help reduce paperwork.  To be completed once or twice a term per child. (If a child has a significant delay/ has SEN needs then more regular observations to track development would be needed)
2. Daily Ratio sheets – ensure you are in ratio at all times with these ratio sheets
3. WOW Vouchers – observations sheets to be sent home for parents/ carers to fill in.
4. Achievement update (final report) – used for when the child is leaving the nursery (usually to go school). A complete report covering the seven areas of learning.
5. Achievement Update (prime and specific areas) – to be completed each term.  Check what areas your children has developed to and use it to set next steps.
6. 2-year-old progress check – a 2-year-old progress check is a legal requirement for every nursery.  Use this to check the your 2-year-olds progress.
7. Nappy change record – record your children’s nappy changes on this document
8. Sleep record – record your children’s sleep times with this document
9. Snapshot Observation – record a quick observation on this sheet
10. Yearly Tracking Sheet – track your children’s  yearly development with this document
11. ABC Behaviour Diary – have a child with challenging behaviour? Attempt to figure out any triggers with this ABC diary
12. Outing Observation Sheet – record the impact and intent of an outing with this document.
13. Sen Referral Guidance – support your staff to understand how SEN referral works with this guidance
14. Early Years Support Plan Blank (SEN support) – Support your children who have suspected special educational needs with this document.  Use this to apply for SEN funding.
15. Individual Education Plan (IEP) – SEN observations and targets – An alternative to the Early Years Support plan above, support your children who have suspected special educational needs with this document.  Use this to apply for SEN funding.
16. Early Identification Signs – Learn the signs of potential sen and what to look out for with this document
17. Daily Sheets – use this simply sheet to give to parents so they know what their children have been up to during the day
18. Characteristics of Effective Learning – guidance to improve the understanding of what the characteristics of effective learning are
19. Next step sheets – set next steps for your children with this document
20. Early Years Outcomes (My Learning Journey) – use this to track children’s development from ages 0 to 60 months.  This document uses the EYFS learning goals covering development in the 7 areas of learning.
